Your character's name by itself is insufficient. You'll have to also say which server and which faction.

I have 4 different ones, so I am guessing for that to happen it had to be some sort of reward.

I am not sure there are any others or a way you can get one since it seems to have been a sub reward and in that case it might not even be able to give to another person as those are normally already bound when you get them. I will see if I make a new character if it is possible but I don't think it will be.

 

Note: It is bound. Tried creating a character and went to check and it says bound.

The "reverse-engineered" part is no longer possible, but you may find people who have the schematics from way back when. I'm not sure if they still drop from Operations.

 

From the PvP vendor, you'll have to go to the vendor (on the Fleet in, I believe, the "Training" section) and see.
