There's some that drop in flashpoints and operations but are bound. There's some that come from quest rewards like running all of the tier 1 and 2 hardmode flashpoints. The one I have on all characters was a reward for referring someone to the game who became a subscriber. There's too many to mention and I'm not aware of any guides.
