I have been having this issue since a little bit before Christmas, I have updated all my drivers, and many of the other troubleshooting ideas. I'm going to list this analysis from an app called razer game booster seen in the first spoiler I know that my system can handle the game b/c I played it without any incidents for almost a year. My problems with the game started back when the patch around dec. 18 2012 was released. For some reason my cpu couldnt install the whole thing. Following the advice of csa for swtor via the phone he had me do some things which didnt seem right but I figured he knew what he was talking about. Fast forward a couple of days still wont install the patch, and I think I forgot to turn back on firewall and some nasty viruses got into my cpu. The result of that was 8 plus hours with microsoft techs over the phone and $100, removed all the viruses but something was still screwy with cpu so I had to reinstall windows vista and everything else from scratch. However after everything was supposedly up to date and I reinstalled the game the patch finished installing but when I press play my computer freezes up and forces me to hard boot my computer.. which means I can't get an error report about the game b/c the last thing the cpu knows is the launcher launched successfully. I am still a paying subscriber amazingly since I havent been able to play for the last month and a half. My firewall has the exceptions for both launcher and swtor.exe.. Please solve this I know it can't be this difficult. If there are other ways I can diagnose what isn't working right let me know how to run those.

[Also here is my dxdiag

Razer Game Booster caused instability in one of my computers I had to put everything back to default and remove the program, it is only a beta after all , not saying this is the cause for you but it caused me grief on that computer.

Have you done the normal first check, like run fix launcher.exe and the game repair function after you login on the launcher. (cog on lower left)

Edit - turn off programs like mumble when you go to start the game

Edit - Please run chkdsk c: /f and sfc /scannow you have a lot of errors outside of swtor occuring

YEAH i NOTICED THAT IT CAUSED SOME INSTABLILITY ON MINE TOO, I ONLY USED IT TO RUN THE DIAGNOSTICS, THEN IMMEDIATELY UNINSTALLED IT. YEAH I'VE DONE BOTH THE FIX LAUNCHER AND REPAIR COG OPTION, HOWEVER NOT ON THIS LATEST INSTALLATION I COMPLETED EARLIER THIS EVENING. IT JUST SUCKS WHEN IT DOESNT WORK BECAUSE HARD BOOTING THE PC ISN'T THE BEST THING FOR IT.

I'm not saying Razer is bad, and it does some things well, but it is a beta and may cause issues for some.

your log shows a lot of access errors , which is why i recommend you run the chksdk and sfc i mentioned above.

What is "mumble" or is that like a genre of programs? Also if nothing comes about from the check dsk or sfc/ scannow would you have any other advice?

Mumble is another program people use with swtor.

Try doing a clean boot http://support.microsoft.com/kb/929135

Make sure McAfees has launcher.exe and swtor.exe in the exception list.

Try turning off programs like PunkBuster that you are currently running in case of conflict.
PnkBstrA.exe

Try turning off indexer

1 > open Search, select the "Indexing" arrow and set it to "do not turn on"
or
2 > run services.msc and double click the "indexing service", set it to disabled and reboot.

Delete DiskCacheArena File then run a defrag before playing again.
