While there is obviously something amiss in the code somewhere, and you are definitely correct that it will take some time to find the problem (especially considering that the responsible devs were likely fast asleep when this hit), this particular incident isn't really about the coding.

Just look at today's maintenance. A properly functioning and diligent QA department would have caught this issue before the servers were brought back up. Matrix Cubes are an extremely common equipped item, therefore the issue should have been ID'd. Then today's maintenance would've been extended. Either the issue would've been fixed before the servers were brought up or else the patch would've been rolled back and the servers brought up in their pre-maintenance state.

From a wider programmatic perspective, allowing a patch to go live with such a glaring error is indicative of a QA program that is lacking in something. Resources, time, proper priorities, adequate testing environments, something. I don't know without reviewing their processes, but these small patches are obviously not being tested enough before being cleared for release.

There is simply no excuse for this kind of thing going live. The fact that Bioware's warning still doesn't state that simply logging in a character (not just "loading in a new area") will cause this issue is only exacerbating the problem.

For the record, I read the warning and, having experience with these types of issues (ahem, experience in a testing environment where it belongs), I refrained from logging in any character that has a Matrix Cube equipped. Therefore, I haven't lost any cubes. My sympathies for those that have lost them, especially if Bioware can't reverse the loss.
