How do I alter my companion's appearance?

I'm a bounty hunter and I'd like to alter Mako. I received the appearance pack, but could not figure it out and inadvertently destroyed it. Did I screw myself? Can I buy different looks somewhere?
I don't like guilds. I once expressed why in my signature, however it was removed and I received a very polite letter asking me to change my mind. I still do not like guilds, however, I'm not allowed to express myself any further than saying that.

You can buy companion kits from certain vendors. There are multiple appearances for most companions. You equip them in a special slot at the lower edge of the companion equipment screen.

I've seen this comment before but cannot find any vendor that sells companion customization. Does anyone know exactly where these vendors are?

Should be one on the fleet station. If not there, its most likely on Coruscant/Dromund Kaas.
