For the past week I have received "Failed Mission" after successfully completing my daily missions in Black Hole and Section X where I had to enter an instance. I put an in-game ticket with CS and they said it was a bug and to report it here. Although it states "Failed Mission" I am still able to turn in at the drop boxes; however, I am waiting for it to say I cannot turn in because of mission failure. Knock on wood, this has not happened yet.
