I don't know if this has been noticed by anyone else, but NPCs aren't loading right for me.

Examples:

Companions don't appear with the slotted gear in conversations.

Extreme example is Risha? from the smuggler story line has on the outfit she gets after you get her as a companion on the ship, but in cut scenes, she is wearing the outfit she's supposed to have.

Then there is later on in places like the Gravestone where background NPCs show up, when they aren't on the ship yet according to the story. (Took screen shots of it)

Is this a known issue? It's not big, but it's disconcerting.
