What do you mean by crazy? Are you asking if he was crazy before he fell to the darkside, or after? If before, no he was just more emotional and unstable letting his feelings get the better of him. When he fell he was more aggressive and hot headed, able to get frustrated though this was more due to the fact he wasn't able to break through Obi-Wan's defense. He could have won, if he didn't do a high jump but he wasn't thinking straight.

Now you wanna see him go crazy? Have a look at Lost Command, Vader went absolutely ape **** near the end when he was trying to find his wife.

Ah well then after, no he wasn't really crazy. He was just more aggressive and not able to think clearly, as far as Fallen Anakin goes. As Vader that is something else.

If you're really interested in Vader's psychological state, I'd recommend James Luceno's "Dark Lord: The Rise of Darth Vader". A lot of his hate was actually directed at himself, but he clearly never gave up (until he saw Luke getting shocked with force lightning while he lay on the floor with, yet again, one arm) on overthrowing the Emperor and taking his place. That possibility was all he had for a while after Padme died. He was extremely frustrated, to say the least, about how weakened he was after he was put in the suit, which he felt (rightly) was more of a prison than a mercy.
In the Imperial Army, it takes more courage to retreat than to advance.

It was a combination of many things that drove him to go a bit nuts by the end of ROTS, later on as a Sith he's perfectly fine, it was him not being at his best emotionally, and the Dark Side wasn't helping any.
