Toth Backhand Smash

We are running Toth and Zorn 16 HM and having an issue that is causing us serious trouble. We are having melee get targeted with backhand smash even though they are standing directly behind Toth. It is a big problem when the person with the yellow circle gets targeted because it will usually kill the target and if it doesn't it puts a lot of stress on our already overtaxed healers. The berserk phase is not the time to be dealing with huge spike damage. If anyone knows what is causing this or how to prevent it, I would appreciate your help with an explanation to prevent it.
This is the tool tip I found on Khira's Blog which has helped a little but not much.

"Backhand Smash A 2 second knockdown. Seems to hit anyone in front of Toth."

Are you sure that Backhand Smash is the problem, and not the stomp? Toth is dealing extra damage this phase, so your melee DPS may need to jump out and wait to get healed. Also, have your healers drop area heals near Toth to cover the tank and whoever has the yellow circle.

Yes, I am very sure that melee is getting the backhand smash. We have had several discussions about it. I know the stomp during berserk causes a lot of damage but the backhand smash is killing the person with the yellow circle. That is what is coming up on the killing blow and people are also getting the knockback from it. We just heal through in SM, but it is too much to handle in HM.

Backhand Smash has a pretty wide arc. Try to make sure that the yellow circle person does not push too far into Toth. Just stay well behind Toth and let the yellow circle barely extend under him.

^^ This - also you don't want the yellow circle too close to the tank, I've noticed the AOE from it is quite a bit wider than the circle and you can take a big chunk of damage when that rock gets tossed over as the tank.

I have noticed that backhand smash seems to have a 180 degree frontal arc with no range. I thought that the person behind Toth was accidently crossing his center dot, but they are telling me they standing almost at max melee range.

You can also have the person with the yellow circle stand further behind Toth, so Toth barely touches the circle, if at all. When it turns purple, the tank can walk into Toth a little. Toth will re-position himself by stepping back into the circle.
