He can die one shot if he has the Breath of the Masters debuff. Or he ended up standing on purple circle as a result of his knockback and breath of the masters. You are vague, you need to explain how this happened and whether the guy who is dead was standing (or pushed to) on a purple circle or he received damage after breath of the masters (he becomes black) -- Note that if his back is towards the wall you won't get knocked back much and as a result kephess may hit you due to short range even if he is taunted (or taunted a bit late). Avoid walls and avoid purple circles behind you when you tank him

Tank swap.

If the second tank taunts right after the knockback then breath of the masters will be applied to the taunting tank and will be one shotted. You have to wait for a second after the knockback to taunt.. In other words, the second you see the breath cast bar you taunt.

 

Combat logs would show your tank got breath and then an arcing slash.

if your tank has his back to a wall after 60% he will get 1 shot. OT taunts during breath of the masters, Standing in purple circles will kill your tank as well.
