From checking your DxDiag it appears that you might not be using the latest available graphics drivers. Please can you go to your cards manufacturers website to make sure you are using the latest versions.

Please also try renaming your Client Settings file(you can just add an extra letter on the end). This will reset your settings so please launch the game, put the settings back to your preferred, and see if the issue persists.
