For anyone with my issue, I used this program as a workaround to force my monitor to not accept any other aspect ratios. My screen in-game is ridiculously stretched, however, the UI is at the correct resolution and everything. The screenshot shows the best example from the login screen (body type 2 btw) (https://imgur.com/gallery/uRZVJPT). I haven't played in a couple of months but had made no hardware or setting changes since last time, and I also play on a 1440p monitor. Drivers and everything are up to date, although threads I've found from as late as 2012 describes this exact issue and that it's likely the game using as incorrect aspect ratio for the monitor instead of using the resolution to determine the correct aspect ratio, leading to 4:3 aspect ratios on monitors that don't support it for example. Following forum reports over the years, this bug seems to pop up every so often due to updates and gets fixed eventually after more updates. This is the only lead on my issue I've found. To confirm, UserSettings and other files in %appdata% have all be edited and played around with, as well as screen settings in Nvidia control panel and in computer display settings with no solution found, as well as uninstalling and reinstalling the game.
