Not 100% sure but I think it unlocks when you have completed the full story for that companion, it doesn't go by affection.

I totally agree, keep the server cap as it is till few days at least after launch, then increase it and offer free character / guild transfers from the higher populated servers to less populated servers. Queue's are a pain but it will only get worse by increasing them before launch.
