I am having the same issue with my Smuggler. Finished all of Chapter 3 couple nights ago and did not get the achievement for completing Chapter 3 - Smuggler. I am guessing it is the same issue to be fixed in the 4.4 patch, at least I hope so. Hate waiting so long to find out though.

When I ran into this a few weeks ago, after patch notes said it was fixed, I found an orphaned companion quest in my quest list. It had a blank title and a blank description if I recall. I abandoned that quest and Ashara's final quest dialog reset for me and I was able to complete her conversations.
